ACCC Travel Insurance Program
Information
Update (September 5, 2006)
The
completed form must be addressed to the ACCC Project Officer assigned
to the project and sent by fax at 613-746-6721 at least two weeks
prior to the traveller's date of departure. Once the request has
been approved, each traveller will be enrolled in the appropriate
insurance coverage program. Proof of enrollment documentation (when
applicable) will be sent to the Project Coordinator (or directly
to the traveller, if requested).
Questions
or concerns should be brought to the attention of the ACCC Project
Officer.
